Renewal Memory Partners provides licensed home healthcare, including skilled nursing and home health aide services. Our team includes registered nurses (RNs), licensed practical nurses (LPNs), and certified home health aides (HHAs). For clients receiving home care, a supervisory nurse is on call and available whenever services are provided.
Our skilled nursing services include:
Assessment and supervision of care
A comprehensive health assessment and personalized Plan of Care created in collaboration with a client's health care team
Medication management
Preparation, organization, and monitoring of prescribed medications
Specialized care
Diabetes care, immunization/injections, wound care, and other skilled nursing services
In addition, our home health aides can assist with
the following:
Personal care
Feeding, bathing, dressing, toileting
Movement and mobility
Walking, getting into and out of bed, as well as doing prescribed exercises
Light housekeeping
Dusting, tidying, and laundry
Meal preparation
Breakfast, lunch, dinner, and light snacks
Errands
Grocery shopping, and sundries
Escorts
Accompaniment and transportation assistance for medical appointments and social outings
Most clients receiving home health care require physical assistance with one or more activities of daily living. If clients are physically independent but still in need of some assistance, they may be candidates for companion care.
