Care Management
As an interdisciplinary corps of social workers, nurses, and Certified Dementia Practitioners, our Care Management team provides round-the-clock support to our clients and their families. Ideal for clients with complex care needs, Care Management encompasses a wide range of support services designed to help older adults thrive.
SCOPE OF SERVICES
On-Site Advocacy:
Bedside support at hospitals, rehab facilities, and other institutional settings.
Physician Liaison:
Accompaniment to doctorsí appointments, managing physician correspondence and MyChart communications, reviewing test results, and providing referrals to geriatricians, neurologists, and other specialists.
Crisis Intervention:
24/7 telehealth support from Renewalís team of registered nurses.
Financial:
Coordination with daily money managers who can assist with bill-paying and other day-to-day administrative needs.
Senior Housing Transitions & Home Modifications:
Referrals to senior housing specialists and/or collaboration with home modification professionals, senior move managers, and downsizing experts.
Insurance, Benefits, & Entitlements:
Navigating long-term care insurance claims, as well as eligibility for various entitlement programs.
Allied Health:
Arranging physical therapy, occupational therapy, speech therapy, massage therapy, music and art therapy, psychotherapy, and other supportive health services.
Life Enrichment:
Curating programs, events, and experiences to enhance quality of life, such as museum outings, concerts, lectures, senior center & social day program referrals.
Family Support:
Counseling, education, and ongoing emotional support to family members and loved ones.
Legal:
Referrals to elder law attorneys for various legal needs, including advance directives, wills, and trusts.
RN Wellness Visits (NY Only):
Medication refills, vital signs, assessing changes in condition, and determining any emerging medical needs.
